When the access group is set to "USER," the accessibility tool fails to work in the Portal

The accessibility tool works as expected in the Pega caseworker portal when logged in with administrator access, but there are issues when logged in as a regular user.

what difference in the behavior you see in both the cases ?
And also please mention the version of the platform you use

You need to add SysAdm4 access role to the regular user access group.
